Benefit eligibility and common life changes

Employees working 20 or more hours/week are eligible for benefits. Eligible dependents are defined as spouse/domestic partner, child(ren)/child(ren) of domestic partner.

Note: Ex-spouses/domestic partners are NOT eligible to be covered

During Open Enrollment

You can make changes to your benefits during Open Enrollment, which takes place each year in October/November for a January 1 effective date.

Life status change

In general, you cannot enroll in, change, or cancel your benefit plans during the year, unless you have a corresponding Life Status Change. For plan purposes, you are considered to have a Life Status Change in the event of:

  • your marriage or divorce;
  • the death of your spouse, former spouse, domestic partner, or child;
  • the birth or adoption of a child;
  • the employment or termination of employment of your spouse, former spouse or domestic partner;
  • the switching from part-time to full-time employment of your spouse, former spouse or domestic partner;
  • you, your spouse, former spouse or domestic partner taking an unpaid leave of asence;
  • a significant change in your coverage that is attributable to your spouse’s, former spouse’s or domestic partner’s employment;
  • a dependent’s involuntary loss of other coverage;
  • a dependent’s move into your plan’s service area; or
  • a court’s order for you to cover a child under a Qualified Medical Child Support Order (QMCSO—see below for details).

Your election to enroll in, change, or cancel your benefit plan coverage will go into effect as of the date of the change, provided that you make this election within 30 days of that date of the change.

  • In addition, you or your eligible dependent may enroll for medical coverage within 60 days after either of the following events:
  • You or your dependent are eligible under a state Medicaid plan or state children’s health insurance program (CHIP) and the Medicaid or CHIP coverage is terminated.
    You or your dependent becomes eligible for a premium assistance subsidy under a state Medicaid plan or CHIP.

Congratulations on your marriage! You can add your new spouse and their dependents (if applicable) to your benefits, or you can cancel coverage if you are enrolling in your new spouse’s plan.

This life status change allows you to enroll in or change these benefits:

To make a change to your benefits you must do the following within 30 days of the date of your marriage: Your change takes effect as of your date of marriage. If you miss the 30-day period, the next time you will be eligible to make a change is during Open Enrollment.

Step 1

Access Workday from any computer with Internet access. Use your work login and password.

To enroll dependents, you’ll be required to upload documentation into Workday indicating the effective date of your marriage., which you will see as you go through the online enrollment process.

Step 2

Upload a copy of your marriage certificate in Workday.

Other important things to consider

Beneficiary Updates: While you can change your beneficiaries for your life insurance and your retirement plan at any time, it’s always a good idea to update this information when you have a life status change.

  • 401(k) Retirement Plan and HSA (if applicable): Fidelity Investments; call 800-343-0860 or log into netbenefits.com/point32health

Name Changes: If you change your name due to your marriage, log into Workday and click on the Personal Information icon, to edit/change your Legal Name.  Be sure to enter the correct effective date.  You will be required to upload documentation to validate your legal name (such as SSN card).  If you have any changes in professional licensure, please provide copies to HR.

To change your name on your Point32Health email and phone extension, go to Compass (using Firefox) to submit an IS Request

Congratulations on the new addition to your family! You can add your new child to your benefit plans and make changes to your existing benefits if necessary.

It is important that you make any change to your benefits within 30 days of the birth/adoption of your baby. Your change takes effect as of date of birth/adoption. If you miss the 30-day period, the next time you will be eligible to make a change is during Open Enrollment.

You must upload a copy of the following documents to Workday within 30 days of the birth or adoption:

  • Birth notice from hospital
  • Birth or adoption certificate

Other important things to consider

  • You may be eligible for six weeks of New Parent Pay.*
  • You may be eligible for a leave of absence*
  • You may be eligible for short-term disability benefits

Definition of “children”

For Plan purposes, your children include your legally adopted children, children placed with you for adoption prior to legal adoption, and stepchildren and foster children who depend on you for support and maintenance. Your children also include any other children:

  • For whom you have legal guardianship;
  • Children of a qualified domestic partner who meet the IRS definition of your dependents; or
  • Children for whom the Company is required to provide coverage under a Qualified Medical Child Support Order (QMCSO – see below).

The age limit for children defined is age 26

  • Coverage for your dependent will automatically end at the end of the month in which your dependent turns age 26. Payroll deductions for medical, dental, and vision, and child life insurance for dependent coverage will end automatically as applicable;
  • You will receive a Workday Alert to inform you that your dependent will automatically be dropped from coverage, there is no action you need to take in Workday to drop the dependent from coverage;
  • Notification of the child dependent’s rights to continue coverage under COBRA will be sent to your home address on record by the middle of the month that your dependent turns age 26;
  • Refer to this chart for COBRA rates;
  • If you think COBRA may not be the right option for your dependent, you may want to consider the Commonwealth Health Connector for their health insurance needs (for Massachusetts residents only). Contact the Health Connector for information and enrollment at http://www.MAhealthconnector.org or 1-877-623-6765. For employees outside of Massachusetts, go to healthcare.gov.
  • Depending on which plan you choose, your monthly premium with the Health Connector could be lower than other coverage options. Unlike COBRA coverage, Health Connector coverage does not have a maximum period; you can continue coverage as long as you pay your premiums on time. Drawbacks: Benefits may not be the same as the coverage through Point32Health. If your dependent enrolls in Health Connector coverage, they are waiving their right to elect health insurance under COBRA.
     

Disabled dependent child

For Plan purposes, a disabled dependent child is your child who:

  • Became permanently physically or mentally disabled before his or her 26th birthday (or, if earlier, two years after the date that the child loses IRS Code dependent status);
  • Is incapable of supporting himself or herself due to disability;
  • Lives with you or your spouse/domestic partner; and
  • Was covered under your family coverage immediately before reaching age 26.

If your dependent child should be listed as disabled, please reach out to the Member Services Team at the number found on the back of your medical ID card to complete the required documents.

You can add coverage for yourself and dependents within 30 days of your legal separation or divorce. You are required to drop coverage for your ex-spouse since they are no longer considered a qualified dependent under Point32Health benefits.

QDRO: If your divorce entails a Qualified Domestic Relations Order (QDRO) for your 401(k) plan, contact Fidelity Investments directly to initiate this process.

If your spouse loses coverage due to loss of job, change of benefits, etc., you are eligible to add coverage through Point32Health for yourself, spouse and dependents. If your spouse gains coverage due to new employment, new benefits eligibility, etc., you eligible to drop coverage from Point32Health for your spouse and dependents.

It important that you report your change within 30 days of when your spouse loses or gains coverage. Your change takes effect as the date of loss/gain. If you miss the 30-day period, the next time you will be eligible to make a change is during Open Enrollment.

Upload a copy of proof of your Life Status Change event to Workday. Examples include but are not limited to letter from spouse’s employer, benefits confirmation statement, COBRA notice, etc.

In general, you cannot enroll in, change or cancel your benefit plans during the year unless you have a corresponding Life Status Change. In those situations, you must make any change to your benefits within 30 days of the event. The change takes effect as of the event date.

Here are examples of Life Status Change events:

  • your marriage, divorce or legal separation;
  • birth, adoption or placement for adoption of a child;
  • your dependent child reaches maximum age limit under the plan or becomes eligible under another plan;
  • death of your spouse, former spouse, domestic partner or child;
  • open enrollment or your spouse or domestic partner’s open enrollment;
  • gain or loss of other coverage;
  • dependent’s involuntary loss of other coverage;
  • employment or termination of employment of your spouse, former spouse or domestic partner;
  • reduction of hours or unpaid leave of absence for you or your spouse;
  • significant change in your coverage that is attributable to your spouse’s, former
  • spouse’s or domestic partner’s employment;
  • dependent’s move into your plan’s service area; or
  • a court order for you to cover a child under a Qualified Medical Child Support Order
