When it comes to improving health care, we recognize that collaboration is key — and that physician leaders are invaluable agents of change in the medical system. Our quality initiatives are founded on collaboration on key topics of mutual interest to improve outcomes and the value of care. We aim to provide you and your teams with the information and support you need to identify and act on opportunities for improvement — to advance our shared goal of empowering members to improve their physical and behavioral health, and overall wellness.
Our Quality Program
Our annually updated Quality Program is guided by our mission to improve the quality and value of the health care for the people and communities we serve.
Direct access to EMR
Granting Point32Health access to Electronic Medical Records (EMR) enables us to have a more complete picture of the member’s care, supports quality reporting, and saves time for your staff.
HEDIS tip sheets
We offer a variety of tip sheets to aid your practice in optimizing scores on HEDIS measures, one of health care’s most widely used performance assessment and improvement tools.
Our Physician Honor Roll
The Harvard Pilgrim Physician Group Honor Roll recognizes physician groups that demonstrate a sustained commitment to providing exceptionally high-quality care to our members.
Harvard Pilgrim’s Quality Advance Program rewards providers for clinical outcomes on a variety of initiatives. We rely on participating providers to submit medical records on certain measures to calculate performance results and program earnings.
We’ve developed a flyer to assist participants in the program in understanding the timeline for submitting this data and the settlement process.
To support our provider partners in identifying opportunities for improvement based on meaningful and actionable information, we offer a Quality Measures report for provider organizations participating in Harvard Pilgrim Health Care commercial products.
The Member Detail Report identifies each patient eligible for the measure (including demographic information), the PCP of the member, and their compliance status (1 indicating compliance). In addition to the member-level record for each measure, this online report also enables the user to drill into the data for selected measures to view underlying compliance status. Data is exportable to Excel to support incorporation of the information into population health workflows.
These reports are available via HPHConnect and are posted to the secure server. For more detail on accessing and using this reporting tool, please refer to the user guide below.
