Ensuring Continuity of care for MassHealth ACO members

When MassHealth launched new accountable care organization models on April 1, 2023, a continuity of care period was established through June 30, 2023 to prevent interruptions in care for members who moved into a new health plan. Since that Continuity of Care period ended on June 30, 2023, we want to provide some important reminders for behavioral health providers:

  • Please be sure to check the eligibility of your patients regularly. You can find information on the member’s plan and eligibility on the Tufts Health Plan secure provider portal (refer to this user guide for assistance on inquiring about eligibility and benefits). Member information is also available on their member ID card.
  • If you were providing care to a member through the Continuity of Care period and are not an in-network provider for the member’s ACO or MCO, please assist the patient in moving their care to an in-network provider. Members can find in-network providers by visiting the Tufts Health Plan provider directory and selecting the applicable plan: Tufts Health Together with Cambridge Health Alliance, Tufts Health Together with UMass Memorial Health, or Tufts Health Together MCO plan. Members can also request assistance by contacting the Tufts Health Together Member Service Department at 888-257-1985.
  • To become an in-network provider for Tufts Health Plan, please follow the instructions on the Join Our Network page.

While Tufts Health Plan is outreaching to out-of-network providers who are caring for members of our ACO plans, as well as members themselves, to ensure a smooth transition of care, we appreciate your support in coordinating the care of our members.
